ABSTRACT

Introduction: Liver biopsy is an invasive procedure with a minimal rate of associated complications, which represents a highly useful strategy for the diagnosis of pathologies in the liver, when the etiology cannot be clarified by non-invasive techniques. It provides information that allows determining the progression and prognosis of liver diseases. Objectives: To evaluate the main etiology causes of liver disease in patients undergoing liver biopsy. Material and Methods: Review of the clinical record of 61 patients who underwent this procedure between 2018 and 2020 at the Hernán Henríquez Aravena hospital, demographic variables, diagnosis that motivates its performance, and associated complications. Results: Average age 58 years, of this 66% were female and 34% were male. The diagnoses that motivated this procedure were: autoimmune hepatitis, liver Tumors, and chronic liver damage of unknown etiology. 100% of the case had a satisfactory sample for the analysis and clarification of the cause of liver disease. 91% did not present complications and of the 8% associated with complications, pain was the most common. Conclusion: Liver biopsy is an effective method that allowed establishing etiology, confirming diagnosis suspicions, and evaluating the progression of liver disease with a low rate of complications.


Introducción: La biopsia hepática es un procedimiento invasivo con una tasa mínima de complicaciones asociadas, que representa una estrategia de gran utilidad para el diagnóstico de patologías a nivel del hígado, cuando a través de técnicas no invasivas no se logra esclarecer la etiología. Además, entrega información que permite determinar la progresión y pronóstico de enfermedades hepáticas. Objetivos: Evaluar las principales causas etiológicas de hepatopatías en los pacientes sometidos a biopsia hepática. Material y Método: Revisión de fichas clínicas de 61 pacientes sometidos a este procedimiento entre el año 2018 y 2020 en el hospital Hernán Henríquez Aravena, se analizaron variables demográficas, diagnóstico que motiva su realización, y complicaciones asociadas. Resultados: Edad promedio: 58 años de estos: 66% eran del sexo femenino y 34% al sexo masculino. De los diagnósticos que motivaron a la realización de la biopsia fuero: hepatitis autoinmune, tumores hepáticos y daño hepático crónico de etiología no precisada. El 100% de los casos tuvo una muestra satisfactoria para el análisis y esclarecer la causa de la hepatopatía. El 91% no presentó complicaciones y del 8% de las complicaciones presentadas, el dolor fue la principalmente descrita. Conclusiones: La biopsia hepática es un método eficaz que permitió establecer etiología, confirmar sospechas diagnósticas y evaluar progresión de enfermedades hepáticas con una baja tasa de complicación.

ABSTRACT

La esofagitis eosinofílica (EoE) es una enfermedad causada por una respuesta inmune frente a antígenos alimentarios en contacto con la mucosa esofágica; por su parte, la enfermedad de Von Willebrand (EVW) es el trastorno hemorrágico hereditario más común en los seres humanos. La característica central de todos los tipos de EVW, es la presencia de cantidades reducidas o de formas anormales del factor de Von Willebrand (FVW) en el torrente sanguíneo. Debido a que no se han reportado casos previos de EVW tipo 2A asociada a EoE, se describe este caso clínico con el objetivo principal de dar a conocer el hallazgo casual de estas dos patologías, la seguridad de la evaluación por endoscopia de vías digestivas altas y el pronóstico de posibles complicaciones


Eosinophilic esophagitis (EoE) is a disease caused by an immune response against food antigens in contact with the esophageal mucosa; alternatively, Von Willebrand disease (VWD) is the most common inherited bleeding disorder in humans. The central characteristic of all types of VWD is the presence of reduced amounts or abnormal forms of VWF in the bloodstream. Since no previous cases of VWD type 2A associated to EoE have been reported, this clinical case is described with the main objective to present the coincidental finding of these two pathologies, the safety of the evaluation by upper gastrointestinal endoscopy, and the prognosis of possible complications

ABSTRACT

Objective: To investigate the representability and etiological diagnostic value of myocardium samples obtained from patients with hypertrophic cardiomyopathy (HCM) by transthoracic echocardiography-guided percutaneous intramyocardial septal biopsy (myocardial biopsy of Liwen procedure). Methods: This study was a retrospective case-series analysis. Patients with HCM, who underwent myocardial biopsy of Liwen procedure and radiofrequency ablation in Xijing Hospital, Air Force Military Medical University from July to December 2019, were included. Demographic data (age, sex), echocardiographic data and complications were collected through electronic medical record system. The histological and echocardiographic features, pathological characteristics of the biopsied myocardium of the patients were analyzed. Results: A total of 21 patients (aged (51.2±14.5) years and 13 males (61.9%)) were enrolled. The thickness of ventricular septum was (23.3±4.5)mm and the left ventricular outflow tract gradient was (78.8±42.6)mmHg (1 mmHg=0.133 kPa). Eight patients (38.1%) were complicated with hypertension, 1 patient (4.8%) had diabetes, and 2 patients (9.5%) had atrial fibrillation. Hematoxylin-eosin staining of myocardial samples of HCM patients before radiofrequency ablation evidenced myocytes hypertrophy, myocytes disarray, nuclear hyperchromatism, hypertrophy, atypia, coronary microvessel abnormalities, adipocyte infiltration, inflammatory cell infiltration, cytoplasmic vacuoles, lipofuscin deposition. Interstitial fibrosis and replacement fibrosis were detected in Masson stained biopsy samples. Hematoxylin-eosin staining of myocardial samples of HCM patients after radiofrequency ablation showed significantly reduced myocytes, cracked nuclear in myocytes, coagulative necrosis, border disappearance and nuclear fragmentation. Quantitative analysis of myocardial specimens of HCM patients before radiofrequency ablation showed that there were 9 cases (42.9%) with mild myocardial hypertrophy and 12 cases (57.1%) with severe myocardial hypertrophy. Mild, moderate and severe fibrosis were 5 (23.8%), 9 (42.9%) and 7 (33.3%), respectively. Six cases (28.6%) had myocytes disarray. There were 11 cases (52.4%) of coronary microvessel abnormalities, 4 cases (19.0%) of adipocyte infiltration, 2 cases (9.5%) of inflammatory cell infiltration,6 cases (28.5%) of cytoplasmic vacuole, 16 cases (76.2%) of lipofuscin deposition. The diameter of cardiac myocytes was (25.2±2.8)μm, and the percentage of collagen fiber area was 5.2%(3.0%, 14.6%). One patient had severe replacement fibrosis in the myocardium, with a fibrotic area of 67.0%. The rest of the patients had interstitial fibrosis. The myocardial specimens of 13 patients were examined by transmission electron microscopy. All showed increased myofibrils, and 9 cases had disorder of myofibrils. All patients had irregular shape of myocardial nucleus, partial depression, mild mitochondrial swelling, fracture and reduction of mitochondrial crest, and local aggregation of myofibrillary interfascicles. One patient had hypertrophy of cardiomyocytes, but the arrangement of muscle fibers was roughly normal. There were vacuoles in the cytoplasm, and Periodic acid-Schiff staining was positive. Transmission electron microscopy showed large range of glycogen deposition in the cytoplasm, with occasional double membrane surround, which was highly indicative of glycogen storage disease. No deposition of glycolipid substance in lysozyme was observed under transmission electron microscope in all myocardial specimens, which could basically eliminate Fabry disease. No apple green substance was found under polarized light after Congo red staining, which could basically exclude cardiac amyloidosis. Conclusion: Myocardium biopsied samples obtained by Liwen procedure of HCM patients are representative and helpful for the etiological diagnosis of HCM.

Introducción: El ameloblastoma es un tumor odontogénico epitelial benigno con tendencia a la recurrencia local si no se elimina adecuadamente. Las alternativas reconstructivas incluyen el uso de colgajos libres microvascularizados, placas y prótesis personalizada de titanio. Objetivo: Describir un reemplazo hemimandibular con prótesis personalizada de titanio posterior a la exéresis de ameloblastoma. Presentación del caso: Mujer de 44 años de edad, que presentó un hallazgo radiográfico durante la realización de tratamiento pulporradicular del diente número 37. Al realizársele el reconocimiento físico facial mostró aumento de volumen en región geniana izquierda mientras que el examen intrabucal detectó expansión de las corticales en la arcada posteroinferior del mismo lado. Se realizó una radiografía panorámica y tomografía axial computarizada con la que se constató la presencia de imagen radiolúcida, multiloculada, en forma de "pompas de jabón" extendiéndose desde el cuerpo mandibular hasta el cóndilo del lado izquierdo. Se tomó muestra para biopsia, con la cual se constató que se trataba de ameloblastoma con patrón folicular. Se realizó abordaje cervical, segmentaria mandibular con margen de seguridad y exarticulación. Se reemplazó la porción eliminada con prótesis personalizada de titanio. Se mantuvo el chequeo posoperatorio en el que se comprobó una buena evolución. Conclusiones: La cirugía constituyó el pilar de tratamiento utilizado. Una vez realizada la resección quirúrgica se reconstruyó el defecto con prótesis personalizada de titanio, proceder de gran novedad en nuestro medio y útil para restablecer la función y estética(AU)


Introduction: Ameloblastoma is a benign tumor of odontogenic epithelium with a tendency to local recurrence if not removed appropriately. Reconstruction alternatives include the use of microvascularized free flaps, plates and customized titanium prostheses. Objective: Describe a case of mandibular replacement with a customized titanium prosthesis after ameloblastoma excision. Case presentation: A case is presented of a female 44-year-old patient who presented a radiographic finding during pulporadicular treatment of tooth 37. Facial physical examination found an increase in volume in the left genian region, and intraoral observation detected expansion of the corticals in the lower posterior arch of the same side. Panoramic radiography and computed axial tomography showed a multilocular radiolucid image resembling soap bubbles which extended from the mandibular body to the left condyle. A sample was taken for biopsy, which confirmed the diagnosis of follicular pattern ameloblastoma. Segmental mandibular surgery was performed by cervical approach with a safety margin and exarticulation. The portion removed was replaced with a customized titanium prosthesis. Post-operative control showed a good evolution. Conclusions: Surgery was the basic component of the treatment applied. Surgical resection was followed by reconstruction of the defect with a customized titanium prosthesis, a procedure of great novelty in our environment useful to restore function and esthetic appearance(AU)

ABSTRACT

Abstract A 72-year-old woman was admitted for acute heart failure. The echocardiography revealed moderate depression of the left ventricular ejection fraction. Coronary disease was excluded by coronarography. Cardiac magnetic resonance showed predominantly left ventricular septal hypertrophy and severe depression of the left ventricular systolic function. There was also a bright, multifocal and patchy late gadolinium enhancement with subendocardial, mesocardial and subepicardial involvement, suggestive of sarcoidosis. Biochemical study, thoracic computed tomography and positron emission tomography were inconclusive for extra-cardiac sarcoidosis. Therefore, an endomyocardial biopsy was performed. The procedure was complicated by the development of complete atrioventricular block, requiring implantation of a cardiac resynchronization pacing device. A few days after device implantation, the patient developed fever. The echocardiography revealed extensive vegetations, and thus the diagnosis of a device-associated infective endocarditis was made. Even though antibiotic therapy was promptly started, the patient ended up dying. Biopsy results revealed lymphocytic myocarditis. This case is paradigmatic because it shows how the etiologic diagnosis of dilated cardiomyopathy can be challenging. Non-invasive diagnostic exams may not provide a definite diagnosis, requiring an endomyocardial biopsy. However, the benefits versus risks of such procedure must always be carefully weighted.

ABSTRACT

Las situaciones de crisis en la sala de operaciones son eventos poco frecuentes, con personal trabajando rápido y bajo presión, intentando sobrellevar el incidente con los menores riesgos para el paciente. Se expone un caso clínico en el marco del trabajo de una unidad de anestesia para cirugía de tórax en la cual se presenta una crisis asistencial abordada por un equipo de trabajo capacitado mediante simulación en la aplicación de la herramienta de "Manejo de los Recursos en Crisis (CRM)". Planteamos que la aplicación de esta herramienta por un equipo capacitado en su utilización contribuyó al éxito y buena evolución de un paciente crítico.


Crisis situations on the operation room seldom happen, with staff working fast and under pressure, trying to overcome the incident at the same time as reducing the risks for the patient. Here we expose a clinical case on the frame of work of an anesthesia unit assembled for thorax surgery during which an asistential crisis occurs and is managed by a group of coworkers trained by means of simulation on the appliance of the tool called "Crisis Resource Management (CRM)". We support the idea that the appliance of this tool by a team of professionals trained on its use, contributed to the success and good evolution on a critical patient.

Abstract Objective: To describe the success rate and the complications after procedures to diagnose abdominal non-Hodgkin's lymphoma in children and adolescents. Methods: A retrospective cross-sectional study was conducted with a population consisting of children and adolescents with abdominal non-Hodgkin's lymphoma diagnosed between September 1994 and December 2012. The sample comprised of 100 patients who underwent 113 diagnostic procedures, including urgent surgery (n = 21), elective surgery (n = 36), and non-surgical diagnosis (n = 56). Results: The most frequent procedures were laparotomy (46.9%) and ultrasound-guided core biopsy (25.6%). The rate of diagnostic success was 95.2% for urgent surgeries; 100% for elective surgeries and 82.1% for non-surgical procedures (p < 0.05). The rates of complication during the three diagnosis procedures considered were significant (p < 0.001; 95.2% of the urgent surgeries, 83.8% of the elective surgeries, and 10.7% of the non-surgical procedures). The length of time before resuming a full diet and starting chemotherapy was significantly reduced for patients who underwent non-surgical procedures when compared with the other procedures (p < 0.001). Conclusion: Non-surgical procedures for the diagnosis of pediatric abdominal non-Hodgkin's lymphoma are an effective option with low morbidity rate, allowing an earlier resumption of a full diet and chemotherapy initiation. Furthermore, non-surgical procedures should also be considered for obtaining tumor samples from patients with extensive disease.


Resumo Objetivo: Descrever a taxa de sucesso e as complicações dos procedimentos para o diagnóstico de linfoma não Hodgkin abdominal em crianças e adolescentes. Métodos: Estudo retrospectivo transversal em uma população de crianças e adolescentes com linfoma não Hodgkin abdominal diagnosticada entre setembro de 1994 e dezembro de 2012. A amostra foi composta por 100 pacientes submetidos a 113 procedimentos diagnósticos, inclusive cirurgia de urgência (n = 21), cirurgia eletiva (n = 36) e diagnóstico não cirúrgico (n = 56). Resultados: Os procedimentos mais frequentes foram laparotomia (46,9%) e biópsia guiada por ultrassonografia (25,6%). A taxa de sucesso diagnóstico foi de 95,2% para cirurgias de urgência; 100% para cirurgias eletivas e 82,1% para procedimentos não cirúrgicos (p < 0,05). Houve diferença significativa entre as taxas de complicação associadas aos três grupos (p < 0,001; 95,2% das cirurgias urgentes, 83,8% das cirurgias eletivas e 10,7% dos procedimentos não cirúrgicos). O tempo decorrido até o reinício da dieta plena e o início a quimioterapia foi significativamente reduzido para os pacientes submetidos a procedimentos não cirúrgicos quando comparados com os outros procedimentos (p < 0,001). Conclusão: Os procedimentos não cirúrgicos para o diagnóstico do linfoma não Hodgkin abdominal pediátrico são uma opção efetiva com baixa taxa de morbidade, permitem uma retomada mais precoce de uma dieta plena e início de quimioterapia. Em pacientes com doença extensa, os procedimentos não cirúrgicos também devem ser considerados para a obtenção de amostras tumorais.

Abstract The renal artery pseudoaneurysm embody a rare vascular complication coming of percutaneous procedures, renal biopsy, nephrectomy, penetrating traumas and more rarely blunt traumas. The clinical can be vary according the patient, the haematuria is the symptom more commom. Is necessary a high level of clinical suspicion for your diagnosis, this can be elucidated by through complementary exams as the eco-color Doppler and the computed tomography scan (CT). This report is a case of a patient submitted a right percutaneous renal biopsy and that, after the procedure started with macroscopic haematuria, urinary tenesmus and hypogastric pain. The diagnosis of pseudoaneurysm was given after one week of evolution when the patient was hospitalized because gross haematuria, tachycardia, hypotension and hypochondrium pain. In the angiotomography revealed a focal dilation of the accessory right renal inferior polar artery, dilation of renal pelvis and all the ureteral course with presence hyperdenso material (clots) inside the middle third of the ureter. The treatment for the majority of this cases are conservative, through arterial embolization, indicated for thouse of smaller dimensions in patients who are hemodynamically stable. However, it was decided by clinical treatment with aminocaproic acid 1 g, according to previous studies for therapy of haematuria. The patient received discharge without evidence of macroscopic haematuria and with normal renal ultrasound, following ambulatory care.


Resumo O pseudoaneurisma de artéria renal constitui-se de uma rara complicação vascular advinda de procedimentos percutâneos, biópsias renais, nefrectomia, traumas penetrantes e mais raramente traumas contusos. A clínica pode variar de acordo com cada paciente, sendo a hematúria o sintoma mais comum. Visto isso, é necessário um alto nível de suspeição clínica para seu diagnóstico, que pode ser elucidado através de exames complementares, como o Ecodoppler colorido e tomografia computadorizada. Neste relato, apresentamos o caso de um paciente submetido à biópsia renal direita percutânea e que, após o procedimento, iniciou com hematúria macroscópica, tenesmo urinário e dor em hipogástrico. O diagnóstico de pseudoaneurisma foi dado após uma semana de evolução, quando o paciente foi internado por hematúria franca, taquicardia, hipotensão e dor em hipocôndrio. Em angiotomografia, foi revelada uma dilatação focal da artéria renal polar acessória inferior à direita, dilatação da pelve renal e de todo trajeto ureteral, com presença de material hiperdenso (coágulos) no interior do terço médio do ureter. O tratamento de escolha para a maioria dos casos é conservador, através da embolização arterial, indicada para aqueles de menores dimensões em pacientes estáveis hemodinamicamente. Entretanto, optou-se inicialmente pelo tratamento clínico com ácido aminocapróico 1 g, com base nos estudos prévios para tratamento da hematúria. O paciente recebeu alta sem evidências de hematúria macroscópica e com ecografia renal normal, seguindo em acompanhamento ambulatorial.

ABSTRACT

Los paragangliomas son tumores neuroendocrinos que surgen de los paraganglios autonómicos extraadrenales, los cuales son pequeños órganos formados por células derivadas de la cresta neural embrionaria con capacidad de secretar catecolaminas. Los paragangliomas están estrechamente relacionados con los feocromocitomas porque son indistinguibles a nivel celular, y a menudo comparten las mismas manifestaciones clínicas, como hipertensión, cefalea episódica, sudoración y taquicardia. El diagnóstico de estos tumores es importante por su riesgo de malignización, por las implicaciones de otras neoplasias asociadas, y para la posibilidad de realizar estudios genéticos para detección de otros casos dentro de una misma familia. El objetivo de este artículo es desarrollar un resumen sobre la epidemiología, manifestaciones clínicas, pruebas diagnósticas y tratamiento de estos tumores. Se presenta el caso de un joven de raza negra diagnosticado de un paraganglioma(AU)


Paragangliomas are neuroendocrine tumors emerging from the extra-adrenal autonomic paraganglia, which are small organs formed by embryonic neural crest-derived cells with catecholamine-secreting capacity. Paragangliomas are closely linked to pheochromocytomas because they cannot be differentiated at cell level and often share the same clinical manifestations such as hypertension, episodic headache, sweating and tachycardia. The diagnosis of these tumors is important because of risk of becoming malignant, the implications of other related neoplasias and the possibility of making genetic studies to detect other cases in the same family. The objective of this article was to make an abstract about epidemiology, clinical manifestations, diagnostic tests and treatment of these tumors. This is the case of a young Black female who was diagnosed with paraganglioma(AU)

ABSTRACT

At present times, general surgeons are continuously faced to evaluate patients with pigmented lesions. Thus, is very important that surgeons acquire adequate knowledge not only to distinguish between suspicious lesion and non suspicious lesion, but also to correctly assess when and how to perform a skin biopsy. The early detection of melanoma and non melanoma skin cancer is one of the most important factors to achieve a better prognosis. The main objective of this article is to provide surgeons some tips and pitfalls to help accurate the evaluation and diagnosis of pigmented lesions. The authors also want to stress out the importance of the team work between surgeons and dermatologist, due that is well documented that multidisciplinary approach to skin cancer raises the possibilities of early diagnosis, adequate treatment and better results for patients with skin cancer.


En la actualidad, el cirujano continuamente se ve enfrentado a evaluar distintos tipos de lesiones cutáneas en los pacientes, por lo que debe tener conocimientos de las características que hacen que una lesión sea sospechosa o no, para evaluar correctamente cuándo y cómo realizar una biopsia de una lesión cutánea. El diagnóstico precoz, tanto del melanoma como del cáncer de piel no melanoma, ha demostrado ser clave para mejorar el pronóstico de nuestros pacientes. Este artículo pretende entregar algunas claves para afinar la evaluación y diagnóstico de las lesiones pigmentadas. Es muy importante también, recalcar la importancia del trabajo conjunto de los cirujanos con los dermatólogos, ya que la evaluación y manejo multidisciplinario mejora sustancialmente el diagnóstico, tratamiento y resultados de los pacientes con cáncer de piel.

ABSTRACT

Infectious complications after transrectal ultrasound-guided prostate biopsy (TRUS-Bx) appear to be increasing, which reflects the high prevalence of antibiotic-resistant strains of Enterobacteriaceae. Identifying patients at high risk for antibiotic resistance with history taking is an important initial step. Targeted prophylaxis with a prebiopsy rectal swab culture or augmented antibiotic prophylaxis can be considered for patients at high risk of antibiotic resistance. If infectious complications are suspected, the presence of urosepsis should be evaluated and adequate antibiotic treatment should be started immediately.

ABSTRACT

Transrectal ultrasound-guided prostate needle biopsy is the ideal method of obtaining prostate specimens for histological analysis and is therefore often used in clinical practice. In most studies, prostate biopsy is considered a safe procedure with few major complications. In the present report, we describe a case of endocarditis with spondylodiscitis, two very rare complications associated with prostate biopsy. In the present report, we describe a case of infective endocarditis (IE) with spondylodiscitis (SD), two very rare complications of prostate biopsy. Only a few cases have been described in the literature reporting IE or SD as complications of prostate biopsy.

ABSTRACT

Small renal masses (SRMs) are defined as radiologically enhancing renal masses of less than 4 cm in maximal diameter. The incidence of renal cell carcinoma (RCC) has increased in recent years, which is mainly due to the rise in incidental detection of localized SRMs. However, the cancer-specific mortality rate is not increasing. This discrepancy may be dependent on the indolent nature of SRMs. About 20% of SRMs are benign, and smaller masses are likely to have pathologic characteristics of low Fuhrman grade and clear cell type. In addition, SRMs are increasingly detected in elderly patients who are likely to have comorbidities and are a high-risk group for active treatment like surgery. As the information about the nature of SRMs is improved and management options for SRMs are expanded, the current role of renal mass biopsy for SRMs is also expanding. Traditionally, renal mass biopsy has not been accepted as a standard diagnostic tool in the clinical scenario because of several issues about safety and accuracy. However, current series on SRM biopsy have reported high diagnostic accuracy with rare complications. Studies of modern SRM biopsy have reported diagnostic accuracy greater than 90% with very high specificity. Also, current series have shown very rare morbid cases caused by renal mass biopsy. Currently, renal biopsy of SRMs can be recommended in most cases except when patients have imaging or clinical characteristics indicative of pathology and in cases in which conservative management is not considered.

ABSTRACT

PURPOSE: Transrectal ultrasonography-guided prostate biopsy (TRUS-Bx) is an essential procedure for diagnosing prostate cancer. The American Urological Association (AUA) Guideline recommends fluoroquinolone alone for 1 day during TRUS-Bx. However, this recommendation may not be appropriate in regions where the prevalence of quinolone-resistant Escherichia coli is high. We investigated the real practice of antibiotic prophylaxis for TRUS-Bx in Korea. MATERIALS AND METHODS: A total of 77 hospitals performing TRUS-Bx were identified and an e-mail was sent to the Urology Department of those hospitals. The questions in the e-mail included the choice of antibiotics before and after the procedure and the duration of antibiotic therapy after TRUS-Bx. RESULTS: A total of 54 hospitals (70.0%) responded to the e-mail. Before TRUS-Bx, all hospitals administered intravenous antibiotic prophylaxis. The percentage of hospitals that used quinolone, cephalosporin, and aminoglycoside alone was 48.1%, 20.4%, and 9.3%, respectively. The percentage of hospitals that used two or more antibiotics was 22.2%. After biopsy, all 54 hospitals prescribed oral antibiotics. The percentage of hospitals that prescribed quinolone alone, cephalosporin alone, or a combination of two or more antibiotics was 77.8%, 20.4%, and 1.8%, respectively. The duration of antibiotic use was more than 3 days in most hospitals (79.6%). Only four hospitals (7.4%) followed the AUA recommendation of a 1-day regimen. CONCLUSIONS: The AUA recommendation was not followed by most hospitals in Korea. This clinical behavior might reflect the high quinolone resistance rate in Korea, and further studies on the most efficient prophylactic antibiotics after TRUS-Bx in Korea are warranted.

Introducción: las lesiones proliferativas son entidades que se presentan en la cavidad bucal. Algunas son de origen traumático y otras son neoplasias. Con frecuencia no son bien diagnosticadas en la clínica, por la similitud que pueden presentarse entre ellas. Objetivo: determinar las características de las lesiones proliferativas diagnosticadas y la coincidencia entre el diagnóstico clínico y el histopatológico. Método: se realizó un estudio retrospectivo de las biopsias procesadas en cinco años en el Departamento de Patología de la Facultad de Estomatología de La Habana atendiendo a edad y sexo del paciente, tipo de lesión, localización anatómica y coincidencia entre el diagnóstico clínico y el histopatológico. Resultados: se encontró que de las lesiones proliferativas, las pseudotumorales aportaron el 18,1 por ciento y las neoplasias benignas el 5,8 por ciento. En el grupo de edad de 10 a 19 años las lesiones pseudotumorales representaron el 7,78 por ciento y las neoplasias benignas el 1,78 por ciento. En el grupo de 60 años o más, las lesiones pseudotumorales aportaron el 22,67 por ciento del total de lesiones proliferativas. El sexo femenino resultó afectado por lesiones pseudotumorales en 61,3 por ciento y las neoplasias benignas afectaron al sexo masculino en un 55 por ciento. La hiperplasia fibroepitelial representó el 54,22 por ciento de las pseudotumorales, mientras que el nevus aportó el 8,67 por ciento entre las neoplasias benignas. Los diagnósticos clínicos de las lesiones pseudotumorales coincidieron en un 50,4 por ciento con los histológicos, para las neoplasias benignas la coincidencia fue del 53,2 por ciento. Conclusiones: las lesiones pseudotumorales predominaron sobre las neoplasias benignas, con un incremento marcado de ambos tipos de lesiones a partir de los 40 años. En el sexo femenino predominaron las lesiones pseudotumorales, y en el masculino predominaron las neoplasias benignas. Se encontró una baja coincidencia entre el diagnóstico clínico e histopatológico para ambos tipos de lesiones(AU)


Introduction: proliferative lesions are entities that are present in the buccal cavity. Some of them are of traumatic origin and others are neoplasias. Frequently, they are not well diagnosed in the clinical practice because of the similarity that can exist among them. Objective: to determine the characteristics of the proliferative lesions diagnosed and the coincidences between clinical and histopathological diagnosis Method: a retrospective study of the biopsies processed during five years in the Department of Oral Pathology of the Faculty of Stomatology of Havana was conducted dealing with age, sex, type of lesion, anatomic location and similarities between the clinical and histopathological diagnosis. Results: it was found that, of the proliferative lesions, the pseudotumoral ones reported the 18.1 percent and the benign neoplasias, the 5.8 percent. In the age group 10-19 years, pseudotumoral lesions represented the 7.78 percent and the benign neoplasias the 1,78 percent. In the group of 60 years or older, pseudotumoral lesions reported the 22.67 percent of the total of proliferative lesions. The female sex resulted to be affected by pseudotumoral lesions in a 61,3 percent and the benign neoplasias effected the male sex in a 55 percent. Fibroepithelial hyperplasia represented the 54.22 percent of the pseudotumoral ones, whereas the nevus reported the 8.67 percent among the benign neoplasias. The clinical diagnosis of the pseudotumoral lesions matched up a 50.4 percent with the histological; for benign neoplasias, the coincidence was 53.2 percent. Conclusions: pseudotumoural lesions predominated on benign neoplasias, with a marked increase in both types of lesions after 40 years. Pseudotumoural lesions predominated in the female sex, whereas the benign neoplasias predominated in the male sex. There was a low coincidence between the clinical and histopathological diagnosis for both types of lesions(AU)

Introducción: el liquen plano bucal es una enfermedad inflamatoria, crónica y recurrente considerada como una respuesta inmune anormal mediada por células T. Uno de los aspectos más importantes es su posible carácter premaligno y el papel de la displasia epitelial. Objetivo: describir los desórdenes celulares característicos de la displasia en las lesiones del liquen plano bucal y su relación con las formas clínicas y otras variables histopatológicas. El universo estuvo constituido por el registro de 14,977 pacientes sometidos a estudio anatomopatológico en el Servicio de Anatomía Patológica en el Departamento de Patología de la Facultad de Estomatología de La Habana, en el período comprendido de 1970 al 2008 del que se extrajo una muestra integrada por 115 pacientes con diagnóstico histopatológico de liquen plano de la mucosa bucal. Se revisaron los archivos de biopsias tomándose aquellas solicitudes con suficientes datos clínicos en las cuales estuviera consignada la presencia de lesiones bilaterales o, en su defecto, más de una lesión distribuida en una o en más de una región anatómica. Las variables clínicas fueron: Liquen plano tipo reticular, atrófico erosivo e hipertrófico y las variables histopatológicas: tipo de queratinización, grosor epitelial, presencia de displasia epitelial. Se realizó observación microscópica de las láminas obtenidas del archivo. Se obtuvieron fotomicrografías. Para las variables cualitativas se calculó la frecuencia absoluta y el porcentaje. Para la estadística analítica se utilizó el test de Chi Cuadrado, aplicando la corrección de Yates, cuando esta era necesaria o bien la Prueba exacta de Fisher. Conclusiones: la presencia de displasia ligera es un hallazgo posible en el liquen plano bucal, más frecuente en el tipo erosivo y que, hasta tanto no se demuestre convincentemente lo contrario, el liquen plano bucal debe ser una lesión tributaria de un seguimiento a corto, mediano y largo plazo(AU)


Introduction: oral lichen planus is an inflammatory, chronic and recurrent disease which is considered an abnormal T- cell mediated immune response. One of the most important aspects is its possible pre-malignant character and the role of epithelial dysplasia. Objective: to describe cellular disorders characteristic of dysplasia in lesions in the oral lichen planus and their relation with clinical forms and other histopathological variables Method: the universe was composed of the registration of 14 977 patients who underwent an anatomopathological study in the Department of Pathology of the Faculty of Stomatology of Havana during the period 1970-2008, from which a sample composed of 115 patients with histopathological diagnosis of oral lichen planus was taken. The records of biopsies were reviewed, taking those requests with enough clinical data in which the presence of bilateral lesions were recorded or, in its absence, more than one lesion spread through one or more than one anatomic region. The clinical variables were: reticular, atrophic, erosive and hypertrophic types of lichen planus, and the histopathological variables were: type of keratinization, epithelial thickness, presence of epithelial dysplasia. Microscope observation of the lamina obtained from the records was performed. Photomicrographs were obtained. For qualitative variables, absolute frequency and percentages were calculated. For analytical statistics, the Chi-square test was used, applying, if needed, the Yates´ correction or Fisher's exact test. Conclusions: the presence of mild dysplasia is a possible finding in the oral lichen planus, which is more frequent in the erosive type and, until proven otherwise, the oral lichen planus should have a short-term, medium-term and long-term follow-up(AU)

OBJETIVO: Avaliar a incidência de complicações pós-procedimento nos pacientes submetidos a biópsia prostática transretal guiada por ultrassom no setor de intervenção do Departamento de Diagnóstico por Imagem da Escola Paulista de Medicina - Universidade Federal de São Paulo.MATERIAIS E MÉTODOS: Foram avaliados, via contato telefônico, 132 pacientes submetidos a biópsia de próstata transretal guiada por ultrassom no período de abril/2011 a junho/2011, seguindo o protocolo padrão do nosso setor.RESULTADOS: As complicações pós-biópsia foram categorizadas em maiores e menores de acordo com a necessidade de avaliação médica adicional. Cinquenta e nove pacientes (61,8%) apresentaram complicações, e desses, grande parte (86,4%) apresentou sintomas leves e autolimitados, considerados menores. Oito pacientes (8,2%) apresentaram complicações maiores, sendo que apenas um deles necessitou de tratamento sob regime de internação hospitalar. A retenção urinária foi a complicação maior mais incidente no nosso estudo.CONCLUSÃO: Corroborando outros estudos da literatura, nosso trabalho demonstrou baixa prevalência de complicações maiores após a biópsia prostática transretal.


OBJECTIVE: To evaluate the incidence of postprocedural complications in patients submitted to transrectal ultrasound-guided prostate biopsy at the Unit of Intervention, Department of Imaging Diagnosis of Escola Paulista de Medicina - Universidade Federal de São Paulo.MATERIALS AND METHODS: Telephone interviews were conducted with 132 patients who had undergone transrectal ultrasound-guided prostate biopsy in the period from April 2011 to June 2011, according to the institution's protocol.RESULTS: Post-biopsy complications were categorized into two groups - minor and major complications, according to their need for further clinical evaluation. Complications were reported by 59 patients (61.8%), most of them (86.4%) with mild and self-limited symptoms, classified as minor complications. Eight patients (8.2%) had major complications, one of which required in-hospital treatment. Urinary retention was the major and most common complication.CONCLUSION: The present study has demonstrated a low prevalence of major complications after transrectal prostate biopsy.

ABSTRACT

Endomyocardial biopsy (EMB) is one of the reliable methods for the diagnosis of various cardiac diseases. However, EMB can cause various complications. The purpose of this study is to evaluate the complication of transfemoral EMB with both fluoroscopic and two-dimensional (2-D) echocardiographic guidance. A total of 228 patients (148 men; 46.0+/-14.6 yr-old) who underwent EMB at Kyungpook National University Hospital from January 2002 to June 2012 were included. EMB was performed via the right femoral approach with the guidance of both echocardiography and fluoroscopy. Overall, EMB-related complications occurred in 21 patients (9.2%) including one case (0.4%) with cardiac tamponade requiring emergent pericardiocentesis, four cases (1.8%) with small pericardial effusion without pericardiocentesis, two cases (0.9%) with hemodynamically unstable ventricular tachycardia (VT), one case (0.4%) with nonsustained VT, one case (0.4%) with tricuspid regurgitation, twelve cases (5.3%) with right bundle branch block. There was no occurrence of either EMB-related death or cardiac surgery. Left ventricular ejection fraction was significantly lower (32.0+/-18.7% vs 42.0+/-19.1%, P=0.023) and left ventricular end-diastolic dimension was larger (60.0+/-10.0 mm vs 54.2+/-10.2 mm, P=0.013) in patients with EMB related complications than in those without. It is concluded that transfemoral EMB with fluoroscopic and 2-D echocardiographic guidance is a safe procedure with low complication rate.
